Smart Funeral Plan. Privacy Policy

1. Definitions
1.1 “The Company” refers to SMART FUNERAL PLAN.
1.2 “The Policyholder” refers to the person who applies for and pays premiums for the
funeral plan.
1.3 “Beneficiary” refers to the person(s) nominated to receive funeral benefits.
1.4 “Premium” refers to the monthly amount payable by the Policyholder.
1.5 “Claim” refers to a request for funeral benefits following the death of a covered member.

2. Eligibility
2.1 The Policyholder must be a South African citizen or legal resident.
2.2 Covered members must meet the age limits as determined by the Company at the time
of application.
2.3 All information provided must be true and complete. Any misrepresentation may result in
cancellation or claim rejection.

3. Commencement of Cover
3.1 Cover shall commence once the first premium has been received and processed by the
Company.
3.2 No benefits shall be payable before cover commencement.

4. Waiting Periods
4.1 A waiting period applies from the commencement date of the policy.
4.2 Natural death claims will be subject to a waiting period as determined by the Company.
4.3 Accidental death may be subject to a shorter or no waiting period, subject to verification.
4.4 No claims shall be paid during the waiting period.

5. Premiums
5.1 Premiums are payable monthly in advance.
5.2 Premiums must be paid on or before the agreed due date.
5.3 Failure to pay premiums may result in suspension or cancellation of cover.
5.4 The Company reserves the right to review premiums with reasonable notice to the
Policyholder.

6. Lapse and Reinstatement
6.1 If premiums are not paid, the policy may lapse.
6.2 Reinstatement may be allowed at the discretion of the Company, subject to outstanding
premiums and conditions.
6.3 Waiting periods may be re-applied upon reinstatement.

7. Benefits
7.1 Funeral benefits are payable in accordance with the selected plan option.
7.2 Benefits may be paid in cash or services, depending on the policy agreement.
7.3 The benefit amount shall not exceed the value stated in the policy schedule.

8. Claims Procedure
8.1 Claims must be reported within a reasonable period after death.
8.2 The following documents are required for a claim: – Certified copy of the death certificate
– DHA-1663 form – Certified copy of the claimant’s ID – Any additional documents reasonably
required
8.3 Claims will be assessed and finalised within a reasonable timeframe once all documents
are received.

9. Exclusions
9.1 No benefit shall be payable if death occurs as a result of fraud, misrepresentation, or
unlawful activity.
9.2 Claims may be declined if information provided at application stage was false or incomplete.

10. Cancellation
10.1 The Policyholder may cancel the policy by providing written notice to the Company.
10.2 No refunds shall be payable for premiums already paid, unless required by law.
10.3 The Company reserves the right to cancel the policy with reasonable notice.


11. Amendments
11.1 The Company reserves the right to amend these Terms and Conditions in line with
regulatory requirements.
11.2 Policyholders will be notified of any material changes.


12. Protection of Personal Information (POPIA)
12.1 Personal information will be collected, stored, and processed in accordance with
applicable data protection laws.
12.2 Information will only be used for policy administration and related purposes.


13. Governing Law
13.1 These Terms and Conditions are governed by the laws of the Republic of South Africa.


14. Acceptance
14.1 By applying for a funeral plan, the Policyholder confirms acceptance of these Terms
and Conditions.
